Liability Waiver & Release
V Academy — Assumption of Risk, Release of Liability & Waiver of Claims

Read this entire agreement carefully. It is a legally binding contract under the laws of the State of Texas. By signing, you give up substantial legal rights, including the right to sue. Do not sign until you understand it.
1. Activities
This waiver covers my participation in all V Academy ("Academy") activities — lessons, open play, leagues, tournaments, events, and use of the facility, tables, and equipment (the "Activities").
2. Assumption of Risk
Table tennis and athletic activity carry inherent risks including falls, collisions, being struck by balls or paddles, strains, fractures, cardiac events, concussion, permanent disability, and, in rare cases, death. I knowingly assume all such risks and participate entirely at my own risk.
3. Release of Liability (Including Negligence)
On behalf of myself and my heirs, I RELEASE AND COVENANT NOT TO SUE the Academy and its owners, staff, coaches, officials, volunteers, and affiliates ("Released Parties") from all claims for any injury, death, or loss arising from the Activities, INCLUDING THEIR ORDINARY NEGLIGENCE, to the fullest extent allowed by Texas law. This does not apply to gross negligence or willful misconduct.
4. Indemnification
I will indemnify and hold the Released Parties harmless from any claim, loss, or expense (including reasonable attorneys' fees) arising from my participation.
5. Umpire & Administration Decisions Are Final
All decisions by Academy umpires, officials, and management — scoring, rulings, disputes, eligibility, and discipline — are FINAL AND BINDING, and I waive any claim arising from them.
6. Right to Remove
The Academy may refuse entry to, suspend, or remove any person at any time for any lawful reason, without refund, and I waive any claim arising from such removal.
7. Health & Medical Authorization
I am physically fit to participate and have no condition that makes it unsafe to do so. I authorize the Academy to obtain emergency medical care on my behalf if needed and accept full responsibility for its cost; the Released Parties are not responsible for any treatment, transport, or its outcome.
8. Fitness & No Impairment
I will not participate while under the influence of alcohol, drugs, or any substance that impairs my ability, and I will stop and notify staff if I feel unwell or unsafe at any time.
9. Rules & Sportsmanship
I will follow all posted rules and staff and coach instructions, treat others with respect, and play safely. I understand unsafe, abusive, or disruptive conduct may result in removal without refund.
10. Facility & Equipment
I will use the tables, equipment, and facility properly and report any damage or hazard immediately. I am responsible for the cost of any damage I cause through misuse or negligence.
11. Personal Property
The Academy is not responsible for the loss, theft, or damage of my personal belongings. I bring all property onto the premises at my own risk.
12. Communicable Illness
I understand that any group or indoor activity carries a risk of exposure to colds, flu, and other communicable illnesses, and I knowingly assume that risk.
13. Insurance
I understand the Academy does not provide health, accident, or medical insurance for participants, and I am responsible for maintaining my own coverage.
14. Media Release
The Academy may photograph or record me during Activities and use my likeness for promotional and educational purposes without compensation, unless I opt out in writing.
15. Minors
If the participant is under 18, the parent or legal guardian signs on their behalf, accepts every term of this agreement for themselves and the minor, and releases and indemnifies the Released Parties to the same extent.
16. Entire Agreement
This document is the entire agreement between me and the Academy regarding the Activities and supersedes any prior or oral understanding. No staff statement changes it, and it remains in effect for all future visits unless I revoke it in writing.
17. Governing Law & Acknowledgment
This agreement is governed by Texas law, with exclusive venue in the courts of Collin County, Texas; if any part is held unenforceable, the rest remains in full effect. I have read and understood this entire waiver, understand I am giving up substantial legal rights including the right to sue, and sign it freely and voluntarily.
New players read and accept this waiver during onboarding. Each signed acceptance is recorded with its version, timestamp, and signature.
